· Conducted the trial on the basis that a wrench, which Mr Mallard had drawn in a picture, was the murder weapon despite making no attempt to prove that Mrs Lawrence’s injuries were consistent with the use of a wrench.

· Failed to disclose forensic test results relating to a wrench to Mr Mallard’s lawyer or ensure police disclosed them.

· Cleared of a further 11 counts of misconduct.

· Failed to disclose to the defence the original statements of witnesses, including forensic testing reports and details of unsuccessful attempts to locate a weapon capable of inflicting the wound found on Pamela Lawrence.

· Cleared of a further 10 counts of misconduct.
